From durable medical equipment and CPAP supplies to wound care, diabetic shoes, and daily living aids, our goal is to help patients live safely and comfortably at home. We are Medicare and Medicaid approved, ACHC accredited, and proudly serve a 90-mile radius from Greenfield with 24/7 delivery and equipment support by trained Clinical Technicians. Our in-store boutique also offers post-surgical garments, mastectomy bras, lymphedema sleeves, and more—because care doesn't stop at the prescription.
Types and Ages of Patients Served:
The team primarily serves adults and geriatric patients with Home Medical Equipment, but there are no age restrictions—services are also available to neonates, children, and adolescents who meet admission criteria.
Admission Criteria:
- Patient/client resides within the geographic area of service.
- Patient/client exhibits service needs congruent with the adequacy and suitability of department personnel and resources.
- Patient/client is under the care of a licensed physician, who will certify to the necessity for home medical equipment as required and if applicable.
- Patient/client or POA consents to treatment or service.
- Patient/client is accepted on the basis of a reasonable expectation that the patient's needs may be adequately met in the patient's place of residence.
- Home must be safe based upon equipment/supplies specifications.
Home Medical equipment safety assessment
Assessment of the patient's medical equipment needs is conducted by a Clinical Technician and may include the following:
- Type(s) of equipment (Make and Model)
- The patient's goals and expectations
- Physician's orders and recommendations
- The home environment (space, electrical safety, and accessibility)
At least annually, information from the QAPI Program, Strategic Planning, Patient/Client Feedback, Community Assessment Surveys, and other valid informational sources shall be assessed to determine if additional services or programs may be implemented or improved to meet patients' needs in or with the cooperation of Home Medical Equipment.
