Types and Ages of Patients Served:
The team primarily serves adults and geriatric patients with Home Medical Equipment, but there are no age restrictions—services are also available to neonates, children, and adolescents who meet admission criteria.
Admission Criteria:
- Patient/client resides within the geographic area of service.
- Patient/client exhibits service needs congruent with the adequacy and suitability of department personnel and resources.
- Patient/client is under the care of a licensed physician, who will certify to the necessity for home medical equipment as required and if applicable.
- Patient/client or POA consents to treatment or service.
- Patient/client is accepted on the basis of a reasonable expectation that the patient's needs may be adequately met in the patient's place of residence.
- Home must be safe based upon equipment/supplies specifications.
Home Medical equipment safety assessment
Assessment of the patient's medical equipment needs is conducted by a Clinical Technician and may include the following:
- Type(s) of equipment (Make and Model)
- The patient's goals and expectations
- Physician's orders and recommendations
- The home environment (space, electrical safety, and accessibility)
At least annually, information from the QAPI Program, Strategic Planning, Patient/Client Feedback, Community Assessment Surveys, and other valid informational sources shall be assessed to determine if additional services or programs may be implemented or improved to meet patients' needs in or with the cooperation of Home Medical Equipment.
Non-invasive ventilation program
Our NIV/HMV program is built around one core belief: a comfortable patient is a compliant patient — and a compliant patient recovers faster and stays out of the hospital.
That's why we've designed our program around frequent, proactive follow-up led by our Licensed Registered Respiratory Therapists (RTs) — not left to chance and not left solely in the hands of the patient or family. Our RTs personally oversee the ongoing care of your loved one in the home, making sure:
- The mask fit is comfortable and appropriate for the patient's face and lifestyle
- Device settings remain clinically appropriate as the patient's condition evolves
- Any comfort adjustments are made quickly, before small issues become reasons to stop using the device altogether
We've found that when patients are genuinely comfortable with their equipment, they use it as prescribed — and that adherence is what drives better outcomes and fewer avoidable hospital readmissions.
