Our Story: Compassion, Care and Community

  The journey to becoming Suburban Hospice illustrates a powerful collaboration among independent hospitals in Indiana. In 2015, Hancock Health partnered with Riverview Health and Witham Health Services to establish Suburban Home Health, enhancing patient care. As a board member, we witnessed the strength of this partnership, based on mutual respect and excellence. Recognizing the need for exceptional end-of-life care, these institutions expanded hospice services, providing vital support to families during challenging times. By pooling resources, they set a remarkable standard in palliative care, ensuring patients receive the dignity they deserve and reinforcing community ties.


While Suburban Home Health and Suburban Hospice operate as separate organizations, they share a close relationship, akin to being “sisters.” Together, we strive to build a seamless “health at home” service line that meets the needs of patients served by hospitals within the Suburban Health Organization (SHO). SHO is a network of independent providers committed to delivering high-value health care services in our communities, ensuring all patients have access to the best possible care. By working together, Suburban Home Health and Suburban Hospice aim to foster compassion and excellence, positively impacting the lives of those we serve.


Suburban Hospice stands on a foundation of trust and a commitment to excellence in end-of-life care. Our team—comprising nurses, social workers, chaplains, aides, physicians, and nurse practitioners—embraces their calling with compassion, ensuring each patient receives personalized support. We strive to create an environment of peace and comfort for patients and their families. It is a privilege to support families during this sacred time, offering medical care and emotional guidance. We are honored to serve the communities that entrust us with their care and to foster relationships built on respect and understanding.

As of 2024, Suburban Health at Home has become a remarkable reality that we are proud to share with our community. This new journey is filled with opportunities for growth and innovation, but Suburban Hospice remains committed to its mission and values. The main aim of hospice care is to provide the best quality of life in the final six months.


For terminally ill patients, pain and discomfort are significant concerns, and at Suburban Hospice, Inc., the aim is to ensure maximum comfort, emphasizing that even small details matter in providing excellent hospice care.

What are the goals of hospice care?

 Hospice care is provided in the patient’s home, a hospice facility, or a similar setting by a dedicated team of professionals, including doctors, nurses, and trained volunteers, who ensure that patients receive comprehensive support. This care addresses physical, emotional, and spiritual needs, offering support for both patients and their families. This care includes: 

  •  Guidance and education on the natural process of dying. 
  •  Pain and symptom management. 
  •  Assistance with emotional and spiritual needs. 
  •  Support for family caregivers.

Suburban Hospice provides care to patients in various central Indiana communities. Our dedicated clinicians have deep connections to the areas they serve and are committed to ensuring their neighbors attain optimal health outcomes.
